In today’s healthcare environment where imaging reimbursement is bundled into diagnosis-related groups (DRGs), hospitals are at financial risk when inappropriate or unnecessary scans are ordered. Seeing ourselves as a long-term partner with our hospitals, RAF was one of the first radiology groups in the country to take the initiative to implement clinical decision support systems (CDSS) such as the CareSelect™ process to reduce the rate of inappropriate tests.

CareSelect™ criteria were developed through the collaboration of over 30 professional societies who determined appropriate imaging for nearly 90% of clinical scenarios.

Radiology Associates of Florida

ACR Select™ clinical decision software was created for use by ordering clinicians. If a test is ordered that has a low appropriateness, a pop-up alerts the clinician of options that are more appropriate. Clinicians are free to go ahead with their original choice, but appropriateness scorecards are sent out each month ranking them in comparison with their colleagues. If the clinician is not sure what type of imaging to order, the system can also help determine the most appropriate tests based upon a patient’s symptoms. Due to the initial success of the clinical decision support system (CDSS) for imaging, Medicare has issued a mandate that it must be available to everyone by January 2020. RAF has implemented this program in outpatient setting since October 2019 well ahead of mandated requirement. Because RAF was the pioneer in implementing CareSelect™, our experience was featured as a case study in the ACR Bulletin.
